Care Case Management & Consultation
LaDelle-Perry Family Services provides case management and consultation services in a social services/health & wellness manner through the use of a Biological-Psychological-Social-Economic status assessment of an individual potential client. Through this assessment, which will be brought together through information from the client, family, friends, social service agencies, health care members and others that may be a part of the care and/or decision making for the client; to bring together a Care Plan that will be a guide in either providing case management services and/or in providing the client with a plan of action in providing short or long term care.

Based on a desire to help Elders and Dependent Adults along with their family, friends and others wishing to improve their well-being and daily living, April LaDelle, M.S. Counseling began LaDelle-Perry Family Services.

We are a part of the community and will be a service of light and well being. Our part is small but the affect is large. A hand in to lift those who need us most.

With our Care Case Management & Consultation Services, we will provide to the community options and solutions for setting up short and long term care, planning for future and immediate care needs, plans for daily living care when leaving a hospital, skilled nursing facility or finding housing options.
Care Case Management is a short term case management service for Elders and Dependent Adults. Through an assessment of needs based on interviews with the Elder or Dependent Adult, family, and others, a Care Plan is developed to assist in setting up care and other possible needs. Care Case Management may range from 1 to 3 months, or 3-6 months
Consultation services are to assist individuals, family, friends, health care, social services or other entities in finding solutions, options and alternatives to issues surrounding the safety, health, well being and daily living of Elders and Dependent Adults. Consultations may take place in a home, skilled nursing facility, board and care, hospital, office or place a client may be residing temporarily or permanently.
